2401:05 so I'd like to turn things over to the head of TED, Chris Anderson,
2501:09 who will introduce today's guest.

2701:17 WPR: Hi Chris. How's it going today?
2801:20 CA: Nice to see you again, Whitney.
2901:21 It's going pretty good here. Amazing days.
3001:25 WPR: That's good. That's great. We have sunshine here in the Northeast,
3101:28 which is nice.
3201:30 CA: So look, I am excited to introduce this guest,
3301:34 because I've known Seth Berkley for a long time.
3401:37 I count him as a friend.
3501:39 He's a man who has really devoted his life
3601:44 to the most profound questions about public health.
3701:49 Vaccines are extraordinary.
3801:51 They save millions of lives.
3901:54 The quest for a coronavirus vaccine
4001:57 is, I think, the biggest single question that the world faces now
4102:02 if we're going to get out of this.
4202:04 So it's just a delight to welcome Dr. Seth Berkley
4302:10 to TED Connects.
4402:13 Come on in, Seth.
4502:16 Seth Berkley: Good to see you there, Chris,
4602:18 and delighted to be with you and all of the TED community.
4702:23 CA: Well, so look, on Tuesday, Bill Gates was here,
4802:26 and he mentioned that your organization, Gavi,
4902:30 is really at the heart of the quest for a vaccine.
5002:35 So tell us a bit -- what is Gavi?
5102:41 SB: So Chris, thank you for that.
5202:43 What's interesting is that 20 years ago --
5302:45 we just celebrated our 20th anniversary --
5402:48 there were all these powerful new vaccines that were being used in wealthy countries,
5502:53 and the challenge is,
5602:54 they weren't getting to the places that they could make the most difference:
5702:58 the developing world.
5802:59 So Gavi was formed as an alliance --
5903:01 WHO, the World Bank, the Gates Foundation, UNICEF -- all working together
6003:07 to try to bring these vaccines to the developing world.
6103:10 And it's been very successful.
6203:12 We've launched 433 new vaccines
6303:15 in the most difficult countries in the world,
6403:17 in the Somalias and Yemens and DRCs and Nigerias.
6503:21 But we've also set up emergency stockpiles for outbreak-based vaccines,
6603:27 so if there's an outbreak anywhere in the world
6703:29 of yellow fever or of things like cholera or meningitis and now Ebola,
6803:34 we have vaccines that are available to do that.
6903:38 And the last thing we are trying to do is build the health systems out
7003:42 to deliver these vaccines,
7103:44 but also to make sure that we can pay attention to new diseases that pop up
7203:49 in different parts of the world.
7303:51 CA: And just give us a sense of the scale of this.
7403:54 How many vaccines do you distribute in a given year?
7503:57 And how many lives do you believe that that may be saving?
7604:01 SB: So, let me give you a macro number.
7704:04 We've immunized more than 760 million additional children --
7804:11 760 million additional children --
7904:13 and prevented more than 13 million deaths.
8004:15 In an average year, we give about a half a billion doses
8104:20 because we started out with six diseases,
8204:22 but now we vaccinate against 18 different diseases.
8304:28 CA: Yeah, the scale of that is incredible,
8404:30 and amidst all the bad news that's happening,
8504:33 it's kind of amazing that this intervention can save so many lives.
8604:39 I mean, help us understand what a vaccine is.
8704:44 SB: So, the original idea, the word "vaccine,"
8804:47 comes from "vaca," or cow.
8904:49 And the observation made in the 1700s
9004:52 was that milkmaids had beautiful skin,
9104:54 whereas everybody else had pockmarks from having gotten over smallpox.
9204:59 And the concept was that she was getting infected with a zoonosis,
9305:03 that is, with naturally occurring cowpox,
9405:06 not smallpox.
9505:07 That then protected against smallpox.
9605:10 And it was tested in those days: Could you artificially do that?
9705:15 They of course didn't understand virology,
9805:17 they didn't understand any of those issues.
9905:19 But what a vaccine is is something that you give
10005:22 to artificially stimulate the immune system,
10105:25 hopefully to not make you sick.
10205:27 But then later on, when the body comes in contact with the real disease,
10305:31 it thinks it's already seen it and it is able to fight it off
10405:35 without making the person sick.
10505:38 CA: I mean, it's kind of a miraculous thing to me
10605:41 that they work that way,
10705:42 that your body is always there looking for these threats.
10805:46 And a vaccine, I guess, the body perceives it as a threat,
10905:50 and therefore arms itself against that threat, right?
11005:55 And that's what gives the protection.
11105:57 So, is that why some people are sort of -- irrationally, I will say --
11206:02 irrationally scared of vaccines
11306:04 and feel that they may be dangerous,
11406:06 because they are a kind of threat that you're putting into your body
11506:10 in a very subtle way?
11606:12 SB: Well, of course, when this first started,
11706:14 there were two ways to make vaccines.
11806:16 You could grind them up and inject them,
11906:19 so-called "whole killed vaccines."
12006:21 So you took organisms and you got an immune response,
12106:24 and sometimes those organisms, even though they were dead,
12206:27 gave you a pretty whopping immune response:
12306:29 your arms were sore, you got fevers.
12406:32 Then we moved to these weakened live viruses,
12506:35 and frankly, those are the best vaccines.
12606:37 That's what measles is.
12706:39 That's what yellow fever is.
12806:40 These are weakened viruses.
12906:42 They don't give you disease,
13006:44 but because they look like the natural viruses,
13106:46 your body gets protection and, frankly, you get protection for your whole life.
13206:51 Today, because people are worried about side effects,
13306:54 we've begun to use molecular biology and use little bits of it,
13406:57 and therefore, it's moved forward.
13507:01 But the reason people are mostly scared
13607:03 is because, frankly, vaccines have been so successful.
13707:06 You don't expect, if you have a child or two children,
13807:08 that those children are going to die of these diseases,
13907:11 unlike in the past, when three or four out of your five or six or seven kids
14007:16 would die.
14107:17 So today, people think, well, gee, these diseases aren't around,
14207:20 they're not that bad and, by the way, if I'm injecting these things,
14307:24 maybe they're not organic, maybe it'll make my child cry,
14407:27 maybe it'll make them sick, and I don't need to do it.
14507:30 And that's the challenge.
14607:31 You don't want to scare people to death on how bad these diseases can be,
14707:34 but at the same time,
14807:36 you want them to understand that these diseases are serious
14907:39 and can cause really bad disease and sequela.
15007:44 CA: So yesterday, you issued a really powerful call
15107:49 for this massive, coordinated global response
15207:52 to tackle the search for a coronavirus vaccine.
15307:57 We're going to come that in a bit,
15407:58 because I think that's a very exciting topic.
15508:03 But I think we need some more background first.
15608:05 I want to go back five years to when you stood on the TED stage
15708:08 and you held up two candidate Ebola vaccines.
15808:13 This was just a few months after Ebola had been terrifying the world.
15908:17 It was basically amazing how quickly those vaccines had been developed.
16008:24 What happened to them?
16108:26 SB: It's a great question,
16208:28 and let me tell a little bit of the story,
16308:30 but at the end, there were two vaccines.
16408:32 One, it turned out, couldn't finish its testing,
16508:35 because the epidemic died down.
16608:38 The other one was fully tested.
16708:40 It had a hundred percent efficacy.
16808:42 We then went on to work with manufacturers to produce that vaccine,
16908:48 at least temporarily,
17008:50 in an investigational form, just in case there were more outbreaks.
17108:53 There were, and those are the vaccine doses
17208:56 that we've used in the DRC.
17308:58 In the last two outbreaks,
17409:00 280,000 people have been vaccinated with this experimental vaccine,
17509:06 and today, there is a licensed vaccine,
17609:09 and we are now procuring a global stockpile of a half a million doses.
17709:16 But let me just say, Chris,
17809:19 the reason they came so quickly at that moment is, after September 11th,
17909:26 there was concern in the US about bioterrorism.
18009:30 Remember, there were anthrax attacks.
18109:32 And so what happened was there was a list of agents,
18209:34 and Ebola, for a short time, was on that list of agents,
18309:37 so people started making vaccines,
18409:39 and later on, they decided that was not necessarily a good bioterrorism agent,
18509:44 so they dropped that off the list.
18609:46 But in the freezers were vaccines that had been started,
18709:50 and they were dusted off,
18809:51 and that's why we could move so quickly in that moment.
18909:54 CA: And yet, how long was it from that moment on the TED stage
19009:57 with the candidate vaccine
19109:59 to actual deployment?
19210:02 SB: So, what happened was,
19310:03 the epidemic began to go down.
19410:06 The clinical trial I told you about was done.
19510:08 It was a heroic clinical trial done by WHO,
19610:11 and it showed that it had these results.
19710:13 That epidemic then stopped.
19810:15 We didn't know if there were going to be more epidemics.
19910:18 It took another number of years to finish the work on the vaccine
20010:22 to make sure it was pure,
20110:23 to figure out how to manufacture it at scale.
20210:26 It's during that period that we put vaccine away
20310:29 and had it available in case there were other outbreaks.
20410:32 And it turned out, there were three outbreaks.
20510:34 One went away quickly, but there were two.
20610:36 I was there on day 13 of the second outbreak.
20710:38 We injected the vaccine, cases went up, then they went down, and controlled it.
20810:42 And then this DRC in North Kivu outbreak,
20910:45 which really was terrible because it was in a war zone.
21010:48 And that's the one where we've been not only vaccinating in DRC
21110:51 but in surrounding countries.
21210:53 By the way, that is now, I believe, day 38 or 39 out of the 42 necessary
21310:59 to say it's over.
21411:01 We hope it is.
21511:02 And that would be, again, an enormous example
21611:05 of what vaccines can do, even in a very difficult setting.
21711:09 CA: And yet, in one way, Seth, it's kind of shocking
21811:13 that the outbreak that happened at the start of 2015, end of 2014,
21911:19 that it happened at all,
22011:20 because the world has known about Ebola for a long time.
22111:23 It's been sequenced and so forth.
22211:25 A vaccine could have been developed and got ready for a possible outbreak.
22311:30 Why didn't that happen?
22411:33 SB: Well, there had been 26 outbreaks before,
22511:36 but each one of them was small --
22611:37 couple of hundred people or a couple of dozen people --
22711:40 in the poorest African countries in the world.
22811:43 There was no market for it.
22911:44 People didn't know how to test it
23011:46 because they would just pop up and then go away.
23111:48 And so even though it was obviously a disease that potentially could spread,
23211:53 it had never really spread before.
23311:55 Of course, in West Africa,
23411:56 they didn't have a good surveillance system;
23511:58 it spread for three months before people identified that it was Ebola,
23612:02 and by that time, it was too late.
23712:03 It had spread.
23812:05 What's important about that lesson
23912:07 is that then caused huge disruption across Africa, across the world,
24012:12 because cases went to other places.
24112:14 And the challenge then was, and the reason we had to step in,
24212:18 was because there still was no market.
24312:21 So the Gavi board said,
24412:22 "We will put out 390 million dollars.
24512:25 We'll put it out there and tell companies, we're open for business,
24612:29 we'll create a market, we'll buy the vaccine."
24712:31 And that led to companies being willing to finish the investment
24812:35 to get us to where we are today.
24912:38 CA: Right, right.
25012:39 So it's a real paradox, right?
25112:41 In a way, the very thing that makes vaccines so extraordinary,
25212:44 that once they're developed, they are so cheap to administer,
25312:49 for a few dollars, I guess, you can administer this dose
25412:53 that will save someone maybe a lifetime of illness
25512:55 or save their lives,
25612:56 and yet so much of medical research and invention and development
25713:02 is done by companies who need to see a revenue stream,
25813:06 and so they don't see it from those tiny little cheap things
25913:09 that might save a lot of lives.
26013:11 So it's a real market failure that in this circumstance now --
26113:15 That's one of the things I guess you're thinking hard about,
26213:18 how on earth do we get round and avoid that market failure
26313:21 crippling the response this time?
26413:23 SB: Well, first of all, one of the reasons Bill Gates likes vaccines is,
26513:26 in a sense, it's a little bit like software creation.
26613:29 You put a lot of money and effort into creating it,
26713:31 but once you've got it, you can produce it pretty cheaply
26813:34 and use it in different places around the world.
26913:36 I don't want to beat up the pharmaceutical industry here
27013:39 because they were heroic in Ebola,
27113:41 but I think realistically, they are for-profit entities,
27213:44 and they have to say to their shareholders,
27313:46 "Somebody's going to pay for this,
27413:48 or we're going to do it as a charitable thing."
27513:50 And if we do it as a charitable thing, they can't keep doing it.
27613:53 Since then, there is a new initiative
27713:55 called CEPI, the Coalition for Epidemic Preparedness Innovations.
27813:58 It was set up at Davos a few years ago,
27914:01 and its purpose is to try to make vaccines
28014:04 for the list of diseases that aren't yet known epidemics
28114:08 but that can potentially be there.
28214:10 And the idea would be using public sector money
28314:13 to get us prepared.
28414:14 Of course, they jumped in on this coronavirus as well.
28514:17 Last thing is, of course,
28614:19 I'm not worried on a coronavirus stage that this is a problem,
28714:24 with not having a market.
28814:25 One of the challenges here is that there may be too big a market for this,
28914:29 and therefore, how do we make sure there's access for developing countries.
29014:33 CA: All right, so talk about this virus, Seth.
29114:35 How is it different from Ebola?
29214:38 How challenging is it
29314:39 to create a vaccine for it?
29414:43 SB: So what's interesting about coronaviruses
29514:45 is that they are animal viruses,
29614:47 probably primarily in bats.
29714:49 They jump into other animals sometimes,
29814:51 and then they jump into humans.
29914:53 So this shouldn't have been a surprise.
30014:55 This is the third coronavirus that has jumped into humans.
30114:59 We had SARS in early 2002,
30215:03 we had MERS a number of years later,
30315:06 and now we have this virus.
30415:08 What's interesting is there is a database
30515:10 that shows there are 30,000-some-odd isolated coronaviruses in animals,
30615:16 and one of the things that people tried to do
30715:18 was say the way these coronaviruses work is they have a spike on them.
30815:21 They're called "corona" because they look like the sun.
30915:24 That spike is where it attaches to a certain receptor in people's lungs.
31015:28 And so somebody said, well, maybe we can begin to look at those spikes
31115:31 and see if they're similar to the human receptors
31215:34 and they can be predicted.
31315:35 But the problem is people don't invest in those types of research.
31415:40 And, of course, I think that,
31515:42 given it's an evolutionary certainty, we're going to see this,
31615:45 that we should be.
31715:46 But one other point about this is
31815:48 coronavirus jumped into humans in ancient history as well,
31915:50 and so we have now about a third to a quarter of the common cold viruses
32015:55 are actually coronaviruses.
32115:57 And what's interesting about those is they don't make you deathly ill
32216:00 like these,
32316:01 but you also don't have long-term immunity to them,
32416:04 so you can get reinfected with these viruses
32516:06 after 10 months, a year.
32616:09 And so that does raise an issue on vaccinology,
32716:12 because you want to ideally have lifetime immunity.
32816:16 CA: The reason why we get reinfected is because the virus mutates slightly,
32916:20 and so it escapes the antibodies?
33016:22 SB: No, no, not in this case.
33116:24 Not in this case.
33216:26 So in flu, that's what happens. The viruses are always mutating.
33316:29 In HIV, the reason we don't have a vaccine is because they're all mutating.
33416:33 In this case, the immune response seems to get weaker and go away,
33516:38 and people get reinfected with the same viruses.
33616:40 Now, that is potentially a solvable problem using vaccinology
33716:44 and many different techniques,
33816:46 but the point is, we just can't assume.
33916:48 Some people now are talking about herd immunity
34016:51 as a way to deal with this virus,
34116:53 and the idea there is if you could get enough people infected --
34216:57 you know, forget for a moment that a lot of people are going to die
34317:00 and be miserable while that happens --
34417:02 but the idea is that you get a certain level immunity in the community,
34517:05 and then the disease will go away.
34617:07 Well, that is only true if you get long-term immunity.
34717:10 If you don't, then you could go through all of that horrible experience,
34817:13 have all those deaths,
34917:15 and then not have the protection you need to protect against this disease.
35017:19 CA: OK, so in a way, the quest we're looking for
35117:22 is a vaccine that will work for the long term.
35217:26 I mean, I guess any vaccine that works at all will be a huge gift,
35317:30 but it could well be one that we have to retake every year,
35417:33 or something like that.
35517:35 SB: Right. That is certainly possible.
35617:37 Of course, we have to remember, though,
35717:39 that SARS and MERS both had even higher mortality than this virus does,
35817:43 and they give a much more profound immune response.
35917:46 So it may be that they react differently than the common cold viruses.
36017:51 The challenge, of course, is that we haven't had the opportunity
36117:54 to study these over a long time,
36217:56 and this new disease -- three and a half months, we've had it.
36317:59 More science has been done for this disease
36418:01 in this short period of time,
36518:03 but we don't understand fully the epidemiology of the virus,
36618:06 the immune response, what's protective, which is the best animal model.
36718:10 All of that is being worked on by science and at breakneck pace,
36818:16 but a lot to learn.
36918:18 CA: So talk about how the medical and the research community responded.
37018:24 Because, the Chinese authorities -- I guess we heard it yesterday --
37118:28 only found out about this sometime in December.
37218:32 Already, early in January --
37318:34 I think the virus started in November, they found out about it in December --
37418:38 by early January, they had already released a sequence of the virus
37518:43 to the world,
37618:44 and now here we are.
37718:45 And I think I saw that more than 40 companies
37818:48 are already claiming candidate vaccines.
37918:51 What does it mean to have a candidate vaccine?
38018:54 Like, have companies tested this already against animals or something?
38118:58 Or are they just looking at a computer model where they go,
38219:01 "That should work"?
38319:03 SB: Well, it's an interesting question you ask there.
38419:05 So first of all, China was heroic on this.
38519:07 They did post the genetic sequence of it.
38619:11 Today, we have companies that can sit down with a computer
38719:15 and from that genetic sequence, make what is a candidate vaccine.
38819:19 Now, a candidate vaccine obviously means it's not a licensed product.
38919:22 It's something that somebody wants to work on.
39019:24 But you're right, you have to have the right nomenclature,
39119:27 because "candidate" can mean I'm working on something, it's in my head,
39219:31 I'm just doing a little work on it, I've got something in a vial,
39319:34 I'm beginning to do testing on it.
39419:37 And so what we saw in that case was a company called Moderna.
39519:41 That was the first vaccine that went into humans.
39619:44 It's a messenger RNA-based system.
39719:46 I actually visited the company, not in this outbreak but before,
39819:49 because the technology is interesting.
39919:51 And what they were able to do was, in 42 days,
40019:54 make a candidate vaccine from the genetic sequence.
40119:57 They didn't need the organism.
40219:59 That now is in clinical testing.
40320:00 Now, there is no licensed mRNA vaccine, so we're going to have to figure out,
40420:06 is it safe?
40520:07 Does it work in different age groups? How are we going to scale it up?
40620:11 All of that.
40720:12 But there are many others who are using conventional vaccinology.
40820:15 An example would be, the French are working on a measles-based vaccine.
40920:20 The idea is to put the spiked protein in the measles vector,
41020:24 and it takes a little bit longer to do that work,
41120:26 but once you have that done, of course, we know how to make measles vaccine.
41220:31 We make hundreds and hundreds and hundreds of millions of doses
41320:35 and provide it to the whole world.
41420:37 If that was to work, that might be easier to scale up.
41520:41 So I think what we want in the race is to have multiple different vaccines
41620:46 moving forward.
41720:47 We don't want one or two.
41820:49 We don't want a hundred in the late stages,
41920:51 because it's expensive and hard to do.
42020:53 But we want to have a diversity of science approaches going forward.
42120:58 CA: Which of the other candidates out there are you excited by
42221:01 or at least intrigued by?
42321:03 SB: Well, for me,
42421:04 the critical issue here is going to be we have to optimize for speed,
42521:09 and so that means, as I said,
42621:11 having examples of all the different new technologies
42721:14 that could potentially work,
42821:15 as well as conventional doses moving forward at the same time.
42921:19 So what you're going to want to do is have this bubble up.
43021:23 And it's not just companies, or big companies.
43121:26 It's also biotech companies.
43221:27 It's also academic researchers that are working on this.
43321:30 You want all of those to bubble up.
43421:32 Then you want to be able to look at what's the most promising,
43521:36 and that will depend upon animal results.
43621:39 It'll depend upon being able to produce those vaccines,
43721:42 have a pathway,
43821:43 and eventually, you will want to put those into human clinical trials.
43921:47 That requires a certain amount of safety work.
44021:50 You can try to accelerate that.
44121:51 But then you need to say, OK,
44221:53 we need to know, do we need one dose, do we need multiple doses,
44321:57 do we need 50 micrograms, 100, 150?
44422:01 Do we need a chemical stimulant we call an adjuvant?
44522:04 Given that this disease,
44622:05 its big problems in outcomes are in the elderly,
44722:11 we might need to put some stimulants in to make it a more potent immune response.
44822:15 So all of that work has to go on. That's what the clinical testing is.
44922:18 Eventually you say, "Aha! Here's the vaccine we're going to use."
45022:22 Now you test it in an efficacy trial.
45122:24 And that is to see, does it work?
45222:26 And at that point,
45322:28 you then have a vaccine that you know works.
45422:30 But there is a stage after that,
45522:32 and that stage is, you've got to work out the manufacturing,
45622:37 have it all worked out so that the regulators know
45722:40 that you can really make this,
45822:41 and that it's pure, it doesn't have any problems with it.
45922:45 And during that period, and that's what we did in Ebola,
46022:48 we were able to use those vaccines to help in outbreaks
46122:52 under a clinical trial protocol while monitoring them and learning.
46222:55 So there's a lot of steps there, and it's complicated,
46322:59 and I've shortened it a little bit.
46423:00 CA: But summarize the steps that they basically need to go through.
46523:04 I heard probably an animal test,
46623:07 and then --
46723:08 SB: Well, for example, Moderna.
46823:10 They went into humans at the same time they're doing animal testing.
46923:14 We don't have a perfect animal model.
47023:16 But normally it takes 10 to 15 years to do this,
47123:20 and that's the compression you're trying to do here.
47223:22 So the challenge is, we can compress all those different clinical trials.
47323:26 The basic way you think about it is, preclinical studies, animals,
47423:30 understanding it, purity, reproducibility.
47523:33 Then you move into human studies.
47623:36 You start off with a small number of healthy people.
47723:38 You then work on the dosing, how much, how often.
47823:41 Then you move into people at risk for the disease --
47923:44 that might be in this case the elderly or people with other conditions --
48023:47 and then eventually do an efficacy trial.
48123:50 Now, one of the cool new things we can do today
48223:52 is something called "adaptive trial design."
48323:54 So rather than do these sequentially,

48924:10 you mean enroll people who have their eyes wide open.
49024:13 They have informed consent. I think that's the term.
49124:16 SB: Absolutely.
49224:18 CA: And they're willing to, I guess, take the risk
49324:21 that this isn't a fully tested vaccine but it may well be efficacious,
49424:27 and so that obviously can help a lot.
49524:31 That's crucial to this, right?
49624:33 SB: Absolutely. They are the unsung heroes of vaccinology,
49724:37 because people go out, they volunteer to take a substance,
49824:40 particularly early on, that don't know how it's going to react.
49924:43 Is it going to make the disease worse? Make it better?
50024:46 Is it going to protect them? Is it going to make them sick?
50124:49 So you try to predict that if you can with animals,
50224:51 but people do do that, and the informed consent says
50324:54 not only you may have these side effects or these problems,
50424:58 but also this vaccine may not work.
50525:00 And so it's important for people to understand that,
50625:03 because you don't want people to go and put themselves at high risk,
50725:06 saying, "Oh, gee, I had a vaccine, and so I'm protected."
50825:09 We don't know that until we get to the efficacy stage of trials.
50925:12 CA: But Seth, even putting together all those dots,
51025:15 what I've heard most people say
51125:17 is that it is likely to take at least 18 months
51225:20 before the world will have a vaccine available
51325:24 at any kind of scale.
51425:26 Is that time line right?
51525:29 And can the world remotely afford 18 months on this?
51625:33 SB: Well, I think, you know, I've given you many questions.
51725:37 I could raise lots more questions.
51825:39 So part of it's going to be luck:
51925:41 How easy is this particular candidate vaccine going to be?
52025:45 How lucky will we be in getting a good immune response?
52125:49 Which approaches will work? Will they be scalable?
52225:51 So I think there's lot of questions there.
52325:54 The world will do everything they can to squeeze it down,
52425:57 but I think that's the time line we're talking about.
52526:00 And remember, it's 10 to 15 years usually.
52626:04 In the case of Ebola, we did it in five years to a licensed product.
52726:07 In this case, we are hoping to squeeze it down dramatically,
52826:11 but there are many things we're going to have to go through,
52926:14 and it's really about making sure that vaccine works and it is safe for use
53026:19 in what ultimately may be billions of people.

53226:24 WPR: Hi. So we have lots of questions coming in, Seth.
53326:27 One of them that's kind of related to this is, you know,
53426:30 a lot of us right now are isolating,
53526:32 and we're not building our exposure to this virus,
53626:34 so how will that affect us in the long term?
53726:37 Will this make us vulnerable to the virus until a vaccine is available?
53826:43 SB: So that's a great question,
53926:45 and, as you know,
54026:47 we don't fully understand the epidemiology of this virus,
54126:50 but there is some sense that there may be asymptomatics.
54226:53 Do they get immune protection?
54326:55 Are they afterwards resistant to infection?
54426:58 We don't know that, but we do know that people do get sick,
54527:01 including young people,
54627:03 and that sickness can be quite severe.
54727:05 Obviously, a lot of it is mild, but some of it is quite severe,
54827:08 and then it gets more severe in the elderly.
54927:10 So I wouldn't recommend that anybody go out
55027:13 and intentionally try to get exposed to this virus now.
55127:17 The whole idea of having isolation now
55227:19 is to try to stop the chains of transmission,
55327:22 protect health workers in hospitals,
55427:25 with the idea being that if you can suppress it enough --
55527:28 and Bill talked about this in his talk --
55627:30 and later on have testing available,
55727:32 you might be able to go back to somewhat of a normal life
55827:35 and then watch for reintroduction of this virus.
55927:39 Of course, at the end of the day,
56027:41 we will probably need a vaccine to be able to completely control that,
56127:45 but the experiment is going on,
56227:47 in China ...
56327:48 Japan has done an amazing job of controlling this
56427:51 with slightly less severe interventions.
56527:54 We've seen in Korea similar things.
56627:57 So the hope would be that if we take it seriously,
56728:00 we actually damp down the exposures and stop this epidemic now,
56828:07 we'll be able to remove to some form of normalcy.
56928:10 And we also may have drugs,
57028:11 and drugs will change the dynamics as well,
57128:13 because people will then know that they are able to get treatment as well.

57728:30 because I get that there are so many things
57828:33 that have to be checked out,
57928:35 but I still worry
58028:38 that the rules are not adapting rapidly enough
58128:43 for the scale of the emergency.
58228:45 I mean, my analogy would be: you're going about your lives,
58328:48 and suddenly there's this emergency,
58428:51 you see that there's this enemy force approaching you from the horizon
58528:54 and coming your direction.
58628:55 You don't, in that circumstance, spend a week trying to test all your guns
58729:01 and make sure they're operating absolutely safely and in the right way.
58829:04 You galvanize and you do take some additional risk
58929:09 for the sake of avoiding the bigger risk.
59029:12 Is that thinking prevalent right now?
59129:16 Are there people trying to make those kinds of trade-offs?
59229:20 How should we think about that?
59329:22 Or do you really believe that the community is galvanizing
59429:25 and moving forward as fast as it humanly can
59529:28 and appropriately balancing the two risks?
59629:32 SB: I think we're seeing heroics in moving forward here.
59729:35 Obviously, you're right, and the reason we talk about going from 10 to 15 years
59829:39 down to something like 18 months
59929:41 is about squeezing those steps as much as possible.
60029:45 The regulators in the Ebola experience were really fabulous.
60129:48 They worked with us
60229:50 and tried to keep any bureaucratic delays down to the smallest amount possible.
60329:55 And I think that's what's going to be important here,
60429:58 is we have to look at every single step
60530:00 and say, "Is it critical?"
60630:01 But you do need to answer a lot of these questions.
60730:04 For example, if you have a vaccine that works in healthy people,
60830:08 it very well may not give an immune response to the elderly.
60930:11 We may need to change that vaccine to make it work there.
61030:15 It may not work in young children.
61130:17 So you need a certain amount of studies done.
61230:20 Of course, if you work in areas that have big outbreaks,
61330:23 you're able to also enroll more quickly
61430:26 and follow people more quickly.
61530:28 That's one of the reasons we'll have to think about this globally,
61630:31 because we don't know in 12 to 18 months,
61730:34 or even six to 18 months, if we're really lucky,
61830:38 where the epidemic will be raging and where we want to do the clinical trials.
61930:42 We should be prepared to do them wherever in the world it's possible,
62030:45 and also do some in different types of countries.
62130:48 Developing countries may have different immune responses
62230:51 than in wealthy countries.
62330:52 CA: What alarms me a bit
62430:55 is that on the models I've looked at,
62530:57 with the possible exception of what happened in China and Japan,
62631:02 by distancing, we can bend the curve, we can reduce infection.
62731:09 But as soon as you go back to normal,
62831:11 there's this huge risk of a massive resurge,
62931:14 and until the vaccine comes along, it feels like your choices are:
63031:18 one, sort of recklessly expose the whole population to the bug
63131:23 and develop some kind of herd immunity,
63231:25 or try and do this scary dance of really cramping down on the economy
63331:31 and all the risks that are associated with that,
63431:35 and risking, if you lift the lid on that, risking these really dangerous
63531:40 second surges.
63631:42 So is that the right way to think about it?
63731:45 There's a scenario where, until this happens,
63831:47 and if it's 18 months,
63931:48 that's an incredibly long time for the world to be
64031:52 in that sort of dangerous, scary dance.
64131:56 SB: Well, I think the issue here is that is a little bit the way to think of it,
64232:00 but the experiment is going on now.
64332:02 China is now releasing its controls, and we will see what happens there.
64432:07 We'll see where they have to clamp back down
64532:09 and what's going to happen,
64632:11 and we'll get a good idea of what that's like.
64732:13 Right now, in many countries, we're still in the upscale period
64832:16 when we're seeing lots of cases.
64932:18 And so we have to break that transmission first
65032:20 before we can have that conversation.
65132:23 I'm the first person that would like a vaccine to occur quicker,
65232:27 and, of course, my job is to underpromise and overdeliver,
65332:34 not the other way around.
65432:35 And I think we have to be careful not to think about,
65532:38 "Oh, we can just have a vaccine in a couple of months."
65632:40 It may be that we're lucky. It may be that it's easy to do.
65732:43 It may be the first few candidates will show promise,
65832:46 we get efficacy, we can scale those up for at least some limited use
65932:49 while it's being worked out.
66032:51 But a lot of things have to fall in place for that to happen.
66132:54 And that's why we want to have an organized, global effort
66232:57 to absolutely incentivize the best possible chance
66333:02 for that to happen in the fastest way.
66433:04 CA: There's some kind of debate out there
66533:06 about whether there might be way, way more cases,
66633:10 mild cases, basically zero-symptom cases of coronavirus out there
66733:15 that may have granted more people immunity than we know.
66833:19 Is that a credible suggestion?
66933:23 More cases and much lower fatality rate than we know,
67033:25 because so many of the cases could be invisible?
67133:28 SB: You know, Mayor Bloomberg used to have a saying that I loved.
67233:31 He said, "In God we trust. Everybody else, bring data."
67333:34 And I think the answer here is we haven't done enough testing to know,
67433:39 and we started out with PCR tests to look for virus,
67533:42 and therefore, if you had recovered, didn't have the virus anymore,
67633:45 we weren't able to pick it up.
67733:47 Now there are beginning to be antibody tests
67833:49 to look to see if you've been exposed
67933:51 and don't have the virus now but have an immune response to it.
68033:54 Once we have those tests operating at scale,
68133:57 we'll be able to understand what the epidemiology is
68234:00 and what's happening,
68334:01 and then we'll be in a much better place to understand how this is playing out.
68434:05 Also, I mean, even the question:
68534:07 We don't see a lot of cases in children --
68634:09 is that because the children get infected but they don't get symptoms,
68734:12 and therefore they might be potential spreaders?
68834:15 Or, is it because those children don't get it at all?
68934:20 CA: So tell us, Seth, about this call that you issued yesterday.
69034:24 I mean, you've said that scientists are behaving heroically.
69134:29 But you've called for something more here
69234:32 from both scientists, companies, governments.
69334:35 Tell us what your call is.
69434:38 SB: So, first of all, I believe that, given the situation here,
69534:41 this is not the time to just let the normal system work,
69634:45 as we've talked about.
69734:46 I think we have to think about vaccines as a global public good.
69834:50 And what that means is that initially, it ought to be public sector financed.
69934:54 Obviously, if others want to contribute resources,
70034:57 I believe they should, fine.
70134:59 But we want to make sure the best approaches come,
70235:02 and it doesn't matter where they come from in the world.
70335:05 We want to make sure if the best approach is in China or Japan or in South Korea
70435:09 or in the US, wherever it comes from, whatever company has the ideas,
70535:14 get them on the table.
70635:15 Then we want a process to say, realistically,
70735:18 how are these being compared?
70835:20 How do we decide which ones are most likely to succeed?
70935:24 And then, as I explained, some diversity in taking those risks.
71035:28 Maybe some new technology, some old technologies to drive forward.
71135:32 Once that happens, then, to try to get clinical trials
71235:35 to drive forward as quickly as possible.
71335:37 Now, the delay here is actually likely to be in manufacturing,
71435:42 because we might want billions and billions of doses,
71535:46 so how do we then begin to invest, at risk, in manufacturing plants?
71635:52 If it's a big company, they may have adequate manufacturing,
71735:55 but we may want to work with contract manufacturers,
71835:57 other companies, or even build plants or use new technologies,
71936:01 modular technologies to do this.
72036:03 And then, of course, at the end is going to be the process
72136:06 of getting the vaccine out to all those who need it,
72236:09 and that's going to need to be dependent upon the risk at that time.
72336:12 CA: Help me understand this better, Seth,
72436:14 because right now, it feels like there's this huge effort going on,
72536:18 but companies are operating, in a way, competitively with each other.
72636:21 To an extent, countries are operating competitively with each other.
72736:26 Are you saying that what the world needs is some kind of widely supported
72836:31 global -- I don't know -- vaccine czar
72936:34 that is pulling together different efforts,
73036:36 coordinating, encouraging everyone to work together for the common good,
73136:41 and trying to get agreement on these big decisions
73236:44 like what are the smart few candidates to get behind,
73336:48 rather than this confusing explosion?
73436:51 And then, how do we coordinate manufacturing, etc?
73536:54 Like, is that a person or a small organization
73636:58 that some combination of governments, WHO, UN needs to put together?
73737:02 SB: So, first of all, you want science to bubble up at the beginning.
73837:06 You don't want to have centralized control,
73937:09 somebody saying, "I know best and I'm going to predict this."
74037:12 So you want it bubbling up from all over the place,
74137:14 but then you want a coordinated effort.
74237:16 The group that is best-placed to do that is the World Health Organization,
74337:20 maintaining a list of all the different programs that are going on.
74437:24 We also have other organizations. I mentioned CEPI before.
74537:27 CEPI has now supported eight different candidates.
74637:30 I think it's going to support more.
74737:32 Right now, WHO has on its list 44 candidates,
74837:35 but some people think as many as double that.
74937:38 So what you want to do is say, which are the most likely to succeed,
75037:42 and then put them through some type of standardized set of criteria
75137:47 to pick a few of them to move forward aggressively for the world.
75237:51 Obviously, science is going to keep moving,
75337:53 they're going to keep changing,
75437:55 and it may be that your original approach isn't right and new ideas may come up,
75538:00 but you do need to have some process of moving this forward.
75638:04 And really, that's what I called for.
75738:05 What we need to make sure is that if companies have adequate resources
75838:10 to do this on their own,
75938:11 that's fine,
76038:13 but if not, they need to be supported by the public sector
76138:15 and, again, making sure there's adequate manufacturing and ultimately distribution.
76238:19 Then, after a period of time, we can go back to normal
76338:22 and return to the normal way vaccines are handled.
76438:27 But I think that's probably the best way to get there.
76538:31 CA: How much might this cost, and who should pay for it?
76638:36 SB: Well, it depends how many cases there are.
76738:38 The good news is, we're talking now about trillions of dollars in economic loss,
76838:44 and this is going to be --
76938:46 CA: I would hardly call that good news.
77038:48 SB: I mean, I'm making the comparison. Sorry, you're absolutely right, Chris.
77138:52 I mean, we're talking about tens of billions of dollars here,
77238:55 not trillions of dollars, and the reason that's important is
77338:59 you want to make sure that any good idea has its best chance of moving forward,
77439:04 and we ought to, again, optimize for speed
77539:08 and not optimize for being cost-effective at this time.
77639:12 CA: So I guess you're saying that, like,
77739:16 the rich countries may well be able to afford some kind of vaccine program.
77839:20 I think what I hear you doing here is representing a lot of the countries
77939:23 that can't afford it,
78039:25 and what you're saying is that the world may have to find a way
78139:28 of spending tens of billions of dollars
78239:30 to avoid trillions of dollars of economic damage
78339:32 and all the hardship that goes with it around the world.
78439:35 Is that about right?
78539:36 SB: That's absolutely right, but I think the important point is,
78639:39 this needs to be a global perspective.
78739:42 I mean, look at what happened with Ebola.
78839:44 We had a vaccine that was originally made in Canada
78939:46 by the Public [Health] Agency of Canada.
79039:48 It was then transferred to a US biotech, then to Merck and Company,
79139:52 which is obviously a global player based in the US,
79239:54 and they're manufacturing it in Germany.
79339:56 That's the way science works,
79439:58 and these vaccines may need components from other places.
79540:01 So how do we think about this in a global way and make sure that --
79640:04 By the way, the second vaccine that's in humans is from China.
79740:07 Of course, they've had a lot of time to work on it compared to some others.
79840:12 And they have a candidate that's moving forward.
79940:14 If that candidate is successful,
80040:16 we want to make sure that's the one that's scaled up.
80140:18 And so for me, it's making sure that we're looking at this
80240:23 as a global ecosystem
80340:25 with the best candidates moving forward for the good of the world.
80440:29 CA: Whitney.
80540:30 WPR: We have a lot of people watching from all over the world
80640:33 and we're seeing questions,
80740:34 especially from some of our friends in India who are watching,
80840:37 connected to this just basically about
80940:39 how are poor nations going to get access to this vaccine?
81040:43 And then, specifically, when we think about who gets the vaccine first,
81140:47 will there be some sort of payment that people are paying for this vaccine
81240:51 and those who can afford it will get access before others?
81340:56 SB: Well, the decision on who will pay for it
81440:58 will ultimately come from the political leaders,
81541:00 and my recommendation would be, you start off as a global public good,
81641:04 you make the vaccine available because we're trying to stop the epidemic.
81741:07 Later on, we can have tiered pricing in different places.
81841:10 But one of the concerns, of course, is:
81941:12 Where is the epidemic going to be at that point,
82041:15 and who needs it first?
82141:16 I would argue that people that will need it first are probably health workers,
82241:20 because health workers are going to be on the front lines
82341:22 and we want them to be there to be able to take care of people.
82441:25 They're at risk of both contracting it as well as spreading it.
82541:28 Then you probably want to think about the high-risk individuals,
82641:31 the elderly, people who have preexisting conditions,
82741:34 and then eventually, the rest of the population, if it's needed.
82841:38 So having some type of way of thinking about that.
82941:41 We'll also need to be thinking about equitable access,
83041:44 and that is going to mean thinking about the entire world.
83141:47 Now, Gavi, in the past, including in India,
83241:50 has worked to make sure these new technologies are there,
83341:53 but these are vaccines that existed and, in this case, it's a new vaccine.
83441:58 We have to make sure that it isn't hoarded only in wealthy countries
83542:03 or in a select set of countries.
83642:05 And one way to do that would be to have vaccine production in multiple places.
83742:09 So today a lot of the vaccines that Gavi uses are made around the world.
83842:13 Some are made in the United States and Europe,
83942:16 but some are made in South Korea, in India, in China, in other countries.
84042:20 And so what we could do is have a vaccine transfer the technology and manufacturing
84142:26 in multiple different sites
84242:27 so we could have enough vaccine for that original launch.
84342:30 But whatever happens, there will always be a period of time
84442:34 when we'll have an exciting vaccine and not enough doses to go around,
84542:38 and that's when we need to take hard decisions
84642:40 based upon science on who should get it.

85042:52 I mean, we still don't have a vaccine for HIV,
85142:55 nor for the common cold.
85242:58 How can you be confident that we can get one this time?
85343:01 SB: Well, first of all, as you know, I did a TED Talk even before the one of 2015
85443:07 talking about HIV and flu and how new science needs to come in,
85543:11 and frankly, we are making progress
85643:13 against some of these incredibly difficult organisms.
85743:16 You talked about variability. That's the problem with HIV.
85843:19 It's constantly changing, and so you're chasing.
85943:22 You get a good immune response,
86043:24 but it's to the strain that was there before,
86143:26 and now you're chasing new strains.
86243:28 There are ways to work around that.
86343:30 It's new science.
86443:31 I, actually, I am optimistic.
86543:33 I'm optimistic because we have some experience with SARS vaccines
86643:36 and with MERS vaccines,
86743:38 and so people have worked on it.
86843:40 They've been able to get good immune responses in animals and in people
86943:44 for those vaccines.
87043:45 And so we can build on that experience.
87143:48 I can't tell you how long they'll last for,
87243:51 how effective they will be.
87343:52 Will they need to have local mucosal immunity,
87443:55 which is in the mouth and nose,
87543:57 as well as serum immunity in the bloodstream?
87644:00 Will they need to have just antibodies or the other arm of the immune system,
87744:04 the cellular arm?
87844:06 These are questions that will need to be answered,
87944:08 but I am a great believer in the power of science,
88044:11 and I think in this case the organism is not going to be quite as difficult
88144:16 as the ones you're talking about that are much more difficult.
88244:20 CA: You mentioned in your TED Talk five years ago, Seth,
88344:23 that we've got this situation where we're spending billions of dollars
88444:27 on nuclear submarines patrolling the oceans
88544:30 for a possible incoming threat, nuclear war threat or whatever,
88644:36 and almost nothing on preparation for a pandemic like the one we're suffering.
88744:43 If the world is adequately shaken up by what's happening now
88844:47 and gets rational about this,
88944:50 what is the key structural shift
89044:53 that would be the pandemic equivalent of having those nuclear submarines?
89144:57 How do we prepare for a new virus
89245:03 that we don't know what it will be or when it will come?
89345:06 How do we prepare to have a much more rapid response?
89445:10 SB: Well, it's a great question and I think the TED community
89545:13 has a role to play here.
89645:14 So first of all, we need better surveillance.
89745:16 We need surveillance everywhere in the world,
89845:18 and that's why we don't want to have another outbreak
89945:21 like in West Africa with Ebola.
90045:22 You want to have a resilient health system
90145:25 in every country that reaches out to the periphery.
90245:27 And that's an important priority.
90345:29 We're doing pretty good with immunization.
90445:31 We've reached 90 percent of the kids of the world
90545:34 with at least one dose of routine vaccine.
90645:36 That's the best of health interventions.
90745:38 But we need to reach that last 10 percent and put that health system in place.
90845:41 Then we need to have a different view.
90945:43 We need to start working,
91045:45 where are our likely hot spots?
91145:47 It's where we're cutting down forest.
91245:50 It's in urban slums, where there's density of populations.
91345:53 It's with climate change
91445:55 and movement of different vectors.
91545:57 And what we need to do is use predictive science,
91645:59 and that's where big data can help,
91746:01 that's where AI can help in terms of trying to do that.
91846:04 And we need to have a one-health approach,
91946:06 because we tend to think of animal diseases --
92046:09 and by the way, people have worked on coronavirus vaccines for animals,
92146:13 because they also cause disease there --
92246:15 we need to make sure that the scientists working on veterinary vaccines
92346:18 are connected to humans, are thinking about those whole ecosystem.
92446:22 And we need to invest in that.
92546:23 And unfortunately, after an epidemic, everybody wants to invest,
92646:29 and they say, "Whatever it takes,"
92746:31 and then we move to on other things, and investments go down.
92846:36 What's different about the military is that there is a baseline of investment
92946:40 that goes on all the time and nobody questions that.
93046:43 It continues, and that level of preparedness seems to be there.
93146:47 Bill Gates in his talk in TED, when we did that back-to-back, said,
93246:51 look, the military are doing war games,
93346:53 they're constantly testing, they have all this preparatory activities.
93446:56 Why are we not doing that in diseases?
93546:58 And, as you know, since then, there has been some war games,
93647:02 and they've basically said we weren't prepared,
93747:04 and I think we're seeing now that, in fact,
93847:06 we're not as prepared as we could be.
93947:08 So my hope, the silver lining would be that we prepare for the next big outbreak,
94047:13 because it's absolutely evolutionarily certain
94147:16 we will continue to have outbreaks.
94247:19 The question is: How prepared are we to deal with those?
94347:23 CA: As we wait for a vaccine,
94447:26 are there other interventions that could be made,
94547:29 for example, the serum from people who have been infected and have recovered?
94647:36 SB: So, that technique has been used in other infectious diseases
94747:39 and throughout history and even in Ebola recently.
94847:42 That's something that could potentially be done.
94947:45 Of course, today, it's more attractive if you can make antibodies in the laboratory
95047:50 and then have those available at scale
95147:52 and use those,
95247:53 and I know of a lot of companies that are working on producing those antibodies,
95347:57 which could be infused in an emergency situation, and do that.
95448:02 Obviously, drugs may play an important role here.
95548:04 There's a similar effort to try to create drugs
95648:07 that are active against this organism,
95748:09 and if we knew that you'd get sick but there was an effective treatment,
95848:13 that would also change a little bit of the dynamics
95948:16 of the fear that exists around this pandemic.
96048:19 So I think there are lots of interventions.
96148:22 Of course, traditionally,
96248:23 a preventive vaccine is the best way to deal with these types of epidemics.
96348:29 CA: So paint us -- it's the inner optimist in me, begging for something --
96448:35 paint us the best case scenario, Seth.
96548:37 Lots of people are saying,
96648:39 we don't want to be huddled in our homes by ourselves for 18 months,
96748:44 lovely though the internet is.
96848:47 What's the best case scenario, putting all the pieces together here?
96948:53 SB: Well, I think what is likely to happen,
97048:56 but I don't want to predict, because we're in unprecedented times here.
97148:59 What's likely to happen is that countries who don't take this seriously
97249:04 will have severe outbreaks,
97349:06 those that really take it seriously
97449:08 and put these extraordinary mechanisms in place
97549:11 will control the disease.
97649:13 It'll take some time, it'll take --
97749:15 you know, this is weeks, maybe months, not years, to do that.
97849:19 And then you end up with a situation where the disease is controlled,
97949:23 and you then can go back to life as normal with some cautions around it.
98049:27 Now there's beginning to be new tests that are going to be home tests.
98149:31 We're going to be able to figure out whether classrooms, regions,
98249:34 countries have disease.
98349:36 We can go back to some level of normalcy,
98449:39 but one thing that's really important is we can't have areas of the world
98549:42 with raging disease.
98649:44 So for example, if people said,
98749:45 "Oh, we're not worried about Africa, we're only worried about our countries,"
98849:49 you might end up in a situation where you have large numbers of infections,
98949:53 the virus is mutating, it's actually adapting to humans.
99049:56 We saw some of that with SARS.
99149:58 And then it is easy to reintroduce.
99250:01 So what you want to do is dampen it down everywhere in the world.
99350:05 And maybe it'll burn out. Maybe that'll be it.
99450:08 I personally think we'll probably need a vaccine,
99550:10 but best case scenario is that those alone will stop the epidemic,
99650:15 and what we'll need is then a vaccine just in case it comes back,
99750:19 but of course if I was a betting man,
99850:21 I'd say let's get a vaccine as soon as possible,
99950:24 because that's the best way to control a viral infection,
100050:27 particularly one that is spread respiratorily.
100150:31 CA: Yeah. It's so interesting what you said there.
100250:33 Like, the vaccine doesn't know what continent it's on,
100350:36 what country it's in.
100450:38 It just does its thing.
100550:40 SB: That's correct.
100650:42 CA: So in terms of people listening here,
100750:45 what kind of psychological advice can you give them?
100850:48 What should expectations be?
100950:49 Like, do we have to be ready to settle in for the long haul here,
101050:53 or should we be looking forward to getting back to business
101150:57 around about Easter time and celebrating?
101251:00 SB: Well, again, I don't want to put a time line on it like others do.
101351:04 What you're going to want to see is that bending of the curve.
101451:07 I think Bill talked about this.
101551:10 You want the reproductive rate below one.
101651:13 You want to get it way below one if you can,
101751:15 and then to begin to see the disease spread.
101851:18 And what you don't want to do is, in the middle of that,
101951:21 jump out and start having parties.
102051:22 It's not time to go on spring break and start mixing again.
102151:25 But with careful control,
102251:28 you can begin to release the controls if that's what science shows us.
102351:32 And I think the most important thing here is we need the data to tell us that.
102451:36 That's why testing is so important.
102551:38 With the wide availability of testing,
102651:40 we'll be able to keep tabs, know what's happening.
102751:43 We'll know how many people are asymptomatic, what's happened.
102851:46 We'll know where there are hot communities,
102951:48 and we'll be able to deal with this,
103051:50 is my prediction.
103151:52 So I don't think this is over a very, very long time,
103251:55 but I wouldn't rush it during this unprecedented moment.
103351:59 Otherwise, we're going to end up seeing what we saw in Italy.
103452:02 What we're seeing in New York right now
103552:04 is the overwhelming of the health system.
103652:07 CA: Yeah, no kidding. New York is a scary place right now.
103752:10 I was out walking today.
103852:12 I hope that was OK.
103952:13 But there was no one. There was no one.
104052:15 Like, you couldn't get within six feet of someone if you tried right now,
104152:19 on the busiest walk spots.
104252:23 That was nice to see, but man, it's startling.
104352:29 SB: I mean, recent data has shown that droplets can spread the disease,
104452:34 and so people are rightfully being cautious.
104552:36 And we didn't know that.
104652:38 You remember when we started, we said it's a point outbreak,
104752:41 out of Wuhan, wet market,
104852:43 you had to be in the market to get the disease.
104952:45 Then it was, if you were with sick people,
105052:47 you got the disease.
105152:48 Then it was maybe asymptomatics.
105252:50 I think as we understand better,
105352:51 that gives us the tools to do the right thing.
105452:55 CA: There's a debate out there that seems to be growing again about masks.
105553:00 The East and West take very different advice on masks.
105653:04 We were hearing from Gary Liu yesterday that everyone in Hong Kong and China
105753:09 is basically wearing masks,
105853:10 and arguably, that has been effective.
105953:12 The advice in the West against not wearing masks --
106053:15 how much of that has been driven by just the fact that there's a shortage of masks
106153:19 and that if anyone needs to wear them, it's medical professionals?
106253:23 If it's water droplets, it seems like masks could be effective in prevention.
106353:30 SB: I mean, the most important intervention, as you know,
106453:33 is some isolation and very careful handwashing or use of sanitizers,
106553:38 because what happens is, you touch your face --
106653:41 I forget the number, I think it's like every one to two minutes --
106753:46 and you touch your eyes,
106853:47 so if you reach a door handle or you have contact with a surface,
106953:50 and we know the virus can live on those surfaces,
107053:53 and then you touch your face,
107153:54 touch your mouth, touch your nose,
107253:56 you can spread it.
107353:57 So the purpose of a mask for a person who is not infected
107454:00 is not so much to keep them from getting infected.
107554:02 It is to keep them from touching their face.
107654:06 So I think what's interesting here is,
107754:08 how do we get people to have this personal sanitation?
107854:12 If we have unlimited quantities of masks, people want to do it as a way
107954:15 to remind themselves not to touch their face.
108054:19 Now, that's very different if you're infected,
108154:21 because if you're coughing,
108254:23 having a mask on does reduce the spread of droplets,
108354:28 and that's why they recommend it in a situation where somebody is infected
108454:34 and has to go to the hospital or has to go out and be seen.
108554:38 CA: Yeah, I was touching my face
108654:40 all through the Bill Gates interview, apparently,
108754:42 and I got called to task by our online friends,
108854:46 which was very nice.
108954:47 I don't know if I've been doing that today.
109054:49 It's funny, you're unconscious of it. It's weird.
109154:51 SB: Yes. No, it's automatic.
109254:53 And in fact, there was a WHO challenge for safe handwashing,
109354:57 and I did a video, and I left my water running while I did it,
109455:00 and my friends in the developing world came to me and said,
109555:04 you know, I live in Switzerland by a lake where we have a lot of water,
109655:08 and I wasn't careful, and they were absolutely right.
109755:11 So it's really good we correct each other,
109855:13 that's an important point,
109955:14 and help each other in doing this
110055:17 to get us to be as compliant as we possibly can for these issues.

110555:33 I think one big question is just for folks who are watching from home

110855:41 How can they contribute to this global response effort?
110955:45 How can they do something to advance this?

111155:50 So first of all, I think it's really important that citizens support leaders
111255:55 who are following science,
111355:56 are using science,
111455:58 because, as you know, sometimes political leaders say,
111556:02 "Well, I don't want to do this because it's not good for my image
111656:05 or it's not good for the economy or it's not good for whatever."
111756:08 And I think you want to have all of your decisions taken by science,
111856:11 understanding that they're not the best science.
111956:13 So, citizens need to applaud,
112056:15 even if it's a tough decision that politicians take for the good
112156:18 based upon science, that's a good thing.
112256:21 The second thing that really would be helpful
112356:23 is this concept: How do we keep our world focused on the fact
112456:28 that these epidemics will occur?
112556:32 Another example I didn't answer,
112656:34 which Chris, when he asked me about what could be done,
112756:37 we talked about this idea of platform technologies.
112856:40 These are vaccines that you can test, get them all ready,
112956:43 and then, when a new organism occurs, you can put it in there
113056:46 and you know how to manufacture it, how to scale it up.
113156:49 These types of things can be done. CEPI is trying to do that now.
113256:52 But the challenge is,
113356:53 if we, a year after this,
113456:55 go down to having no money available for these types of issues,
113556:59 that will be a problem.
113657:00 So what you need is citizens to say,
113757:02 "I understand now
113857:04 that health is precious,
113957:06 and I want my government, my leaders
114057:09 to invest in this, in the science,
114157:12 in the ways of working
114257:13 so that we will be as safe as we can be going forward."
114357:16 And I think I can't emphasize enough how important a message that is
114457:20 for citizens everywhere in the world.

115657:55 what we need is the world to come together at this moment,
115757:58 not to talk about our national programs,
115858:01 not to talk about our science are the best,
115958:03 and they may be the best,
116058:05 but how do we, as a world,
116158:07 pool our best science, our best resources,
116258:10 our best ways of working, our best manufacturing,
116358:13 our best clinical trials,
116458:15 to move this forward as fast as possible?
116558:17 The WHO is a global organization whose job is this normative function,
116658:22 and we can get scientists to help them make sure that the normative function
116758:27 is as strong as it can be.
116858:29 We can get the leaders of the world to come together
116958:32 and put the resources in place.
117058:33 Given the cost it's having on the economy,
117158:35 this will be a real bargain to invest in it.
117258:38 But what we need is to have that mindset of having science drive us
117358:43 and to make sure that we have no barriers in stopping that science going forward.
117458:47 That's my request to the world, and I think we can do it,
117558:51 and if we do that, we will end up seeing the power of science,
117658:54 which will give us the tools we need
117758:56 to either stop it -- hopefully, it'll be stopped by then --
117859:00 but stop it and then prepare us for the next one.
